Solution:

The hybridisation of one $s$, one $d$ and three $p$-orbitals on a central atom gives rise to five $sp^3d$ orbitals, three equatorial and two axial. This hybridisation uses one $d_z^2$, one $s$ and all three of the $p$ orbitals and shape of the molecule becomes trigonal bipyramidal.
