Solution:

Sulphur in $SF _{6}$ is $sp ^{3} d ^{2}$ hybridised.

$d- $ orbitals used are $d _{ x ^{2}- y ^{2}} \& d _{z^{2}}$ (both axial d-orbitals)

wrt option $b,$ phosphorous in $PCl _{5}$, is $sp ^{3} d$ hybridised. Here, $d$ orbital used is $d _{z^{2}}$ (axial).

wrt option $c , Xe$ in $XeF _{5}^{+}$ is $sp ^{3} d ^{2}$ hybridised. $d$ -orbitals used are $d _{ x ^{2}- y ^{2}} \& d _{z^{2}}$ (axial).
