We use maximum particle velocity
$v_{\max } =A \omega=5\, cm / s ; T=4\, s$
$\Rightarrow \omega =\frac{2 \pi}{4}=\frac{\pi}{2}$
$\Rightarrow A =\frac{5}{\pi / 2}=\frac{10}{\pi} cm.$
